To be considered for a YLF delegate spot, your application must include:

  1. Application Form (each section must be completed, pages 1–5)
    1. Personal Information
    2. Description of Disability
    3. School and Community Involvement
    4. Diversity Information (optional)
    5. Additional Information
  2. Two Reference Forms
  3. Essay (responding to three topics)

Your application must be submitted NO LATER than March 27, 2009
